For your convenience, there are now a number of forms in the archive section of the site including the new Patient Leaflet, Prescription Pre-payment form and a Self Certification SC2 form.

- Home Visits
We
welcome requests for home visits from patients who are
housebound or terminally ill. Please try and ring any
requests in before 11am. Some requests for a visit may
be dealy with over the phone by one of the doctors or
you may be advised to attend the surgery and this will
be arranged.
We
recommend that children who are unwell are brought into
the surgery for assessment, as this makes best use of
the doctors time and allows for rapid assessment. If
you feel your child is unwell, and there are no appointments
available on that day, please ask the receptionist to
discuss your concerns with the doctor.

- Immunisation Clinic News
From
10 August 2004, there will be no Tuesday 3 - 4pm Immunisations
Clinic at Long Lane Medical Centre. There will only
be a Baby Clinic, on Thursdays 1 - 3:30pm and by appointment
only.

- Care at the Chemist
What
is Care at the Chemist?
Care at the Chemist is a scheme that allows you to obtain
medicines to treat certain illnesses from local pharmacies
without having to go to the doctor first.
Who
can use it?
Anyone currently registered with Long Lane Medical Centre.
Which
illnesses can be treated?
Constipation or diarrhoea
Headache, earache, temperature
Teething / mouth ulcers
Bites and stings
Hay Fever
Conjunctivitis (Red Eye)
Contact dermatitis
Haemorrhoids
Indigestion, heart burn, tummy upset
Coughs, sore throat, blocked nose
Thrush
Head lice
Athletes foot
Threadworms
Cold sores
How
does it work?
You can take part in Care at the Chemist in two ways.
The practice can give you a referral form to take to
the pharmacy. You can also go straight to the pharmacy.
To join, you will need to show that you are registered
with us. The pharmacist will give you a passport that
can be used next time.
The
pharmacist or his staff will ask you some questions
and decide if you need medication, and which would be
best for you. The pharmacist may decide that you need
to see your doctor, or give other advice without supplying
medicines.
If
they think you need to see a doctor urgently, they will
contact the practice to arrange it.
Can
I still see my doctor?
Yes. Taking part in Care at the Pharmacy does not stop
you seeing your doctor. By going to the pharmacy for
your medication, it means the doctor can see patients
with serious illnesses quicker.
Which
chemists are part of the scheme?
Landford Pharmacy is currently the closest chemist to
the surgery that is involved in the scheme. Landford
Pharmacy is situated at 34 Landford Avenue, Liverpool,
L9 6BR.
